Pre-Sedation Assessment Section
This section allows you to document pertinent information regarding sedation, including:
- The history of any adverse reaction to anesthesia
- The patient's American Society of Anesthesiologists (ASA) Score. If the ASA Score is 4 or 5, then the procedure cannot safely be performed in an outpatient setting.
- The Plan of Care for the patient regarding sedation

In the Intake stage of an encounter, this section appears on the Intake checklist for appointment types that are mapped to the "Structured Procedure Visit" or the "Exam-Only Structured Procedure Visit" encounter layout. You can use the Assign Appointment Types to Layouts page to map appointment types to encounter layouts.

- Click into the Pre-Sedation Assessment section in the Review section of the Exam stage.
Note: You can also access this section in the Intake stage.
A new window appears for you to enter information. - Time — Enter the time you perform the assessment and select AM or PM from the list.
- History of adverse reaction to anesthesia — Select Yes or No.
- ASA Score — Select the ASA Score.
- Plan of Care — Select a plan for the patient during the procedure.
- Patient Sign consent — Select Yes or No.
- Notes — Enter any additional notes.
- Click Save.
